Patients with advanced prostate cancer have a complex disease with increasingly complex management issues. These patients are faced with making multiple decisions regarding clinical care options, understanding and dealing with the financial implications of their care, living with side effects of cancer and the various treatments, handling the emotional aspects of chronic illness and adjusting to living with a terminal diagnosis without giving up. Due to the complexity of these issues, patients will often stop seeking treatment or become passive in their participation in treatment. Both possibilities reduce the patient’s potential quality and quantity of life. Since patients are seeking care from UCA across a spectrum of providers, we need a consistency in assuring that each patient is receiving the care needed in a timely fashion.
The UUC/Oncology Care Team will be comprised of the physicians with a primary focus on advanced prostate cancer care (the “Champions”), the associated mid-level providers, the Navigator, the clerical staff and staff nurses associated with the physicians on the Team. The Director of Clinical Services will have a supervisory role and will act as a consultant and facilitator when coordination of additional clinical services is needed.
To offer outstanding patient care to advanced prostate cancer patients (those on androgen deprivation therapy). By offering a “team approach,” patients will have more ready access to care, improved communication and compliance/adherence. When compliance/adherence falters, these patients will be re-engaged so that they may continue with the next appropriate line of therapy. When non-medical issues present, we will direct patient to appropriate resources to offer holistic care.
MA, LPN or RN with knowledge of the environment and systems in which patients receive urology care.
The successful Navigator will have excellent written and verbal communication skills, exhibit compassion
and patience with the patients. Organizational skills and a detail-oriented focus are needed. Experience in
a Urologic/Oncologic clinical setting is preferred but not required.

DUTIES INCLUDE BUT ARE NOT LIMITED TO:
The Nurse Navigator is a member of the clinical care team of Urology Centers of Alabama. Using a combination of communication strategies, resources and tools, the Navigator will:
· Assist in the initial identification and recruitment of potential patients. This will involve participation in seminars, returning patient phone calls and inquiries, visits to outside provider offices, etc.
· Coordinate patient care including scheduling, care transitions, communication, and referrals for services in collaboration with physicians and the broader care delivery team across the continuum of care.
· Assist effective communication with patients by helping existing patients understand their diagnosis and treatment plan, communicating expectations for their care moving forward and reinforcing the availability of “next line” therapies.
· Serve as the Chronic Care Coordinator for the Prostate Cancer Chronic Care Management patients. Interact with and document these encounters as outlined in the CCM plan.
· Assist in arranging and verifying that each patient has Advance Directives
· Establish and maintain a database that allows systematic tracking of patients. Interactions flowing from this tracking will be communication with internal and external providers regarding key issues, follow-up phone calls (or other contacts) post-appointment or procedure. Contact with those “lost to follow-up” will be key.
· Utilize our data mining software to produce needed reports and coordinate with the database to enable each patient to receive appropriate care.
· Be a point of first contact for existing patients within the UCC/Oncology department with return of phone calls or other messages as a key component.
· Promote awareness of clinical trials to patients, families, and caregivers.
· Build partnerships with local agencies and groups for referrals.
· Document interactions, outcomes, and adherence.
· Maintain strictest confidentiality.
· Be a resource for other UCA Team Members regarding issues related to advanced prostate cancer.
· Performs other related work as required.
ENVIRONMENTAL / WORKING CONDITIONS: Work is performed in a medical office setting. Frequent personal communication and telephone contact with patients. May be stressful at times. Interaction with others is constant and interruptive.
ESSENTIAL JOB FUNCTIONS: May require assisting with patients in exam rooms to dress or undress as well as to get on or off exam tables which results in lifting more than 100 pounds. Requires prolonged walking, standing, and sitting. Repetitive bending and twisting at the waist. May require squatting or kneeling as well as reaching above shoulder height. May required pushing or pulling to assist patients.
